Non-squamous NSCLC in China: an evidence pathway, not a treatment ranking.
What can published sources tell us about stage-specific treatment, and what additional hospital-level outcome evidence would answer a narrower question?
Current state: published sources support separate facts about surgery, a China-approved drug regimen, and ECMO use. No hospital-specific non-squamous NSCLC outcome dataset has been purchased or analyzed for this case. No patient-specific success probability is established.

China non-squamous NSCLC evidence pathway
PUBLIC RESEARCH PREVIEW · PERSONAL ESTIMATE NOT ESTABLISHED
Claim
The assembled public evidence can rank surgery, drug therapy, transplantation, and ECMO for an individual with non-squamous NSCLC in China.
Scope
China non-squamous NSCLC; clinical stage, resectability, EGFR status, prior treatment, and fitness determine which evidence is relevant. The defined drug-trial cohort is EGFR-mutated, locally advanced or metastatic disease after EGFR-TKI progression.
Time boundary
Knowledge cutoff 2026-09-28; public research preview v0.1; clinical interpretation review pending.
Current bounded state
PUBLIC RESEARCH PREVIEW · PERSONAL ESTIMATE NOT ESTABLISHED
Supports
Separate stage-specific surgical guidance and an operated cohort, a China-approved regimen and randomized trial result for its defined population, and an ECMO utilization count.
Does not support
A patient-specific success probability or cross-modality ranking. ECMO is life support, not tumor therapy; transplantation is exceptional and not established for the indexed advanced post-TKI cohort. No hospital-specific outcome dataset has been acquired for this case.
Next minimum verification
For the public research gap, request a documented written response on availability and permitted aggregate analysis of a defined hospital cohort; this would not by itself yield an individual forecast.
A citation is incomplete if “Does not support” is omitted. Derived from the published research preview. No hospital data were purchased or analyzed and no clinical interpretation review is claimed.

Clinical context first
Different pathways answer different questions.
These rows do not describe interchangeable options for one patient. Resectability, stage, molecular findings, prior treatment, and clinical fitness change the relevant evidence.
Resectable early or selected locally advanced NSCLCSURGERY EVIDENCE
Chinese lung-cancer guidance describes stage-specific surgery. A published Chinese multicenter cohort reports outcomes after surgery for stage IA–IIIA NSCLC; it is an operated cohort, not a comparison with advanced post-TKI drug patients. National Health Commission guidance · Surgical cohort.
Local treatment / radiotherapySTAGE-DEPENDENT
Record local-treatment eligibility and outcomes separately by stage and intent. The case has not assembled a complete radiotherapy cohort. Chinese guidance.
EGFR-mutated, locally advanced or metastatic, after EGFR-TKI progressionTRIAL RESULT
China approved ivonescimab with pemetrexed and carboplatin for this defined indication in 2024. In the 322-person randomized HARMONi-A trial, final median overall survival was 16.8 months with ivonescimab plus chemotherapy and 14.1 months with chemotherapy control; HR 0.74 (95% CI 0.58–0.95). These are trial-group results, not an individual forecast. NMPA approval · 2026 final OS report · Challenge this gap.
Lung transplantationEXCEPTIONAL CONTEXT
International candidate guidance limits consideration for cancer to very selected lung-limited, low-invasive adenocarcinoma patterns. It does not establish transplantation as an alternative for the advanced post-TKI trial cohort. ISHLT selection consensus.
ECMO during critical illness or a procedureLIFE SUPPORT
ECMO supports circulation or oxygenation; it does not treat the tumor. A Chinese study counted 2,073 ECMO cases in 1,700 tertiary hospitals in 2018, but its authors could not resolve outcomes for specific diseases from those records. These are not non-squamous NSCLC success figures. Chinese ECMO study · ELSO guidance.
One directly randomized comparison
What the drug trial measures.
HARMONi-A recruited adults with EGFR-variant, non-squamous, locally advanced or metastatic NSCLC after EGFR-TKI treatment at 55 Chinese sites. The 2026 final report used an April 2025 data cutoff.
OVERALL SURVIVAL
16.8 vs 14.1 months
Median overall survival for ivonescimab plus chemotherapy versus placebo plus chemotherapy in this trial. Median follow-up: 32.5 months. Read the final report.
SAFETY
67.1% vs 54.7%
Grade 3 or higher treatment-emergent adverse events in the same groups. The outcome and adverse-event definitions are distinct. Read the final report.
APPLICABILITY
Still a clinical question
The trial does not establish that its median applies to a particular person, another treatment line, surgery, transplantation, or ECMO. Understand population statistics.
Next paid evidence question
Ask for treatment effects and feedback at cohort level.
The following is a proposed procurement specification, not a dataset currently held by StructEvidence.
Hospital-controlled analysis
Exposure → follow-up → outcome
Within a defined non-squamous NSCLC cohort: stage, EGFR status, prior TKI, treatment line, regimen, relative treatment dates, dose changes, discontinuations, adverse events, overall survival from a specified start date, censoring, and completeness of follow-up.

Aggregate results with methods
Eligibility and exclusions, denominator, treatment patterns, Kaplan–Meier curves with numbers at risk, time-point estimates and uncertainty, missingness, competing care, data provenance, and limits on publication. No patient-level timeline is requested for the public case.
Data status at v0.1: NOT ACQUIRED. Availability, price, analysis rights, publication rights, and ethics/privacy review require a documented written response from the data holder. Silence or refusal is recorded as an access outcome, not as proof that the treatment has no effect.
If asked “What is my probability of success with surgery, a drug, transplant or ECMO?”, this case stops: the pathways have different eligibility and endpoints. It can show each source's population, outcome and uncertainty, then direct the person to a treating multidisciplinary team for clinical interpretation and shared decision-making.
